    Assist our clients in navigating the health care system and collaborating with the health care team.   The position will work closely with our clients and members promoting wellness, problem-solving and helping individuals realize their personal goals.  The role includes patient outreach, assessments, and documentation. This position is part of a team focused on achieving the goals of the Case Management program.


    • Provides case management services to members with chronic or complex conditions
    • Contacts members who are eligible for the program for enrollment and obtaining basic health history and information regarding the member
    • Identifies, assesses and manages members per established criteria
    • Performs ongoing monitoring of the plan of care to evaluate effectiveness
    • Develops a coaching relationship with members and assists them through the process of actively working towards better health by identifying personal goals, providing support, encouragement and education
    • Documents contacts, assessments and care plan progress in information system
    • Responsible for supporting operational aspects of the division to meet the organization's customer requirements and satisfaction
    • Coordinate care with other health-related vendors for optimal patient care
    • Maintain confidentiality related to all computer programs, medical records and other data


    • Credentialing requirement – at least one of the following, Registered Nurse, Clinical Social Worker (LCSW), Certified Case Manager, or Clinical Professional Counselor (LCPC). Must have current license or certificate in the State of Illinois
    • Education: Bachelor's Degree in nursing, social work, or related field
    • Three or more years of clinical case management, including experience in motivational interviewing
    • Ability to ensure completion of projects and assignments
    • Must have ability to work independently and meet deadlines
    • Possesses, software knowledge including word processing and spreadsheets
    • Highly skilled in verbal and written communication
    • Able to problem solve, exercise initiative and make decisions
    • Computer skills including MS Word, Excel, Access, Outlook, etc.
